Family members on membership: Must be a spouse or child under 18 years or full time student living at residence. (Children under the age of 12 are Not allowed in the fitness center unless in a TLAC supervised class. Children ages 12-16 must work out under the supervision of parent or guardian, or TLAC staff and are required to go through the orientation process to receive an appropriate workout.)

TERMS OF Membership

1. The term of this agreement is 12 months. Monthly dues for the term of this agreement shall be calculated upon your first visit to TLAC. Upon expiration of the term of this agreement, TLAC may adjust the monthly dues.
2. Joining fees are not refundable. Memberships are non-proprietary, non-voting, and non-transferable.
3. Members may bring guests only in accordance with the Club's rules and regulations. Members shall be responsible for the conduct of their guests and the payment of any charges incurred by those guests.
4. The Member may terminate his or her membership after the expiration of the term set forth above by notifying the Club in writing 30 days prior to termination date and paying all amounts then owed to the Club in full. Notice of termination many be given in writing or by completing forms in the Club's business office.
5. Unless the Member terminates his or her membership as set forth in the preceding paragraph, the membership will be automatically renewed on a month-to-month basis at the expiration of the term set forth above. The Member will be obligated to pay monthly dues, regardless of whether he or she uses the Club facilities, until 30 days after the Club has received written notice of termination.
6. Management of the Club may suspend or cancel the right, privileges, or membership of any member whose actions are detrimental to the enjoyment of the Club facilities by other members.
7. Club management may, from time to time, change the rules and regulations governing the operation of the club. Notice of these changes will be made available to members through normal means of communication.
8. The Member will be liable for payment of all costs incurred by the Club in the collection of past due obligations to the Club. Including court costs and reasonable attorney's fees.
9. Should the Member permanently move from his or her current residence as listed on this agreement and more than 25 miles from TLAC, payment on this agreement may be canceled upon receipt of Members new address and date of move. Member shall be relieved from the obligation of making payment for services other than those received prior to move, and if Member has prepaid any sum for services the unused portion shall be promptly refunded.
10. The Member acknowledges and accepts the risks inherent in the use of Club services and facilities. By use of the Club facilities and services, the Member hereby voluntarily assumes the risk of injury, accident, death, loss, costs, or damage to his or her person or property, which might arise from the use of the Club facilities or services. The Member, his or her, executors, representatives, or assigns, hereby releases the Club from all claims or liabilities for personal injury or property damage of any kind sustained by the Member while on the premises of the Club. The Member further certifies that he or she is in good physical health and is able to undertake and engage in physical exercise or sports activities in which he or she chooses to participate.
11. The purpose of this agreement is to insure that the Member understands the duties and responsibilities of Club membership. This is a legally binding agreement and if the Member does not understand it, he or she should consult an attorney of his or her choice.

I (We) hereby apply for membership at Tipton Lakes Athletic Club. I (we) certify that I (we) have read and understand the terms of this agreement as well as the Club rules and regulations, and agree to abide by such rules and regulations.

You, the buyer, may cancel this agreement by midnight of TLAC's third business day after the date of the agreement, excluding holidays. Such cancellation must be in writing, sent certified mail to Tipton Lakes Athletic Club, 4000 Goeller Boulevard, Columbus, IN 47201. Such cancellation must be postmarked by the aforementioned time.
